個人情報保護に関する声明
MFAジャパン株式会社(以下「当社」)は、当社あるいは当社傘下の指導員（以下「指導員」）が催するMEDIC First Aid応急救護講習 プログラム（以下「コース」）を導入、あるいは受講なさるお 客様（以下「クライアント」）の個人情報の機密性および安全性を保護し、 そのための継続的な見直しと取組みを行うことが重要であると考えます。
当社が、クライアントから入手する個人情報は: 主としてクライアントの住所、氏名、受講日、受講したプログラム名、担当指導員名であり、継続教育コースを受講する場合には、 以前に受講した（他社コー スを含む）コース名と受講日の取得が必要になることがあります。これらの個人情報は講習の開始に先立って、 あるいは講習が終わるまでに、適法かつ公正な手段によって担当指導員により収集され、後日当社に提出されます。
当社は、「個人情報保護法」に定義される個人情報を保護するため、その基本的な取組み方針についてこの個人情報保護に関する声明(プライバシー・ステートメント)を策定し、 下記のように、クライアントのプライバシーを保護することを誓約します。

当社は、次に掲げる利用目的の達成に必要な範囲内に限り、クライアントの個人情報を取り扱います。
- 記録のため: どなたが、どのコースを、いつ、どこで、どの指導員の下で受講したかを記録し、クライアントに対するアフターサービスや当社内における検討資料として使用するため。
- クライアントご自身の修了カードの有効期限が近づいてきたことをお知らせするためのご案内ハガキ発送のため。
- 開催されているコースの質を確保・維持するために当社が行う「Quality Assurance(質の確保)プログラム」の一環として行うランダム調査のため。この場合の目的は消費者保護です。
- 教材開発やサービスの向上のため当社が行う市場調査・データ分析のため。
- 善き隣人賞に応募頂いたクライアントご自身とのコミュニケーションのため。善き隣人としての体験談を当社のホームページに開示する場合にはクライアントご自身の同意の下に行います。
- 安全な地域社会を究極の目的として行う応急救護法の普及のために、コースを導入頂いたクライアントの企業名、組織名、団体名、あるいは施設名を当社のホームページに掲載させて頂くため。その場 合には、あくまでもクライアントご自身のご賛同の下に行います。
- クライアントご自身によるお問い合わせやカード再発行依頼を含む様々なリクエストに応えるため。
- クライアントご自身への継続教育コースや新プログラム、あるいは市民レベルの救助に有益な商品のご案内を含むクライアントご自身への営業またはマーケティング活動を行うため。
- その他、当社およびクライアントご自身の担当指導員がお客様とのお取引とアフターサービスを適切かつ円滑に履行するための一切の目的。
クライアントの個人情報を、前述の利用目的の達成に必要な範囲内において、クライアントの担当指導員が指導担当者として使用する場合があります。 この場合、当社は、クライアントの個人情報の安全管理をはかるため、関連する法令およびガイドライン等に基づき、当該指導員に対して必要かつ適切な監督を行うよう務めるものとします。
当社は、司法または行政当局から開示を求められた場合を除き、クライアントの個人情報をクライアントご自身の同意なく第三者に開示または提供いたしません。 また、傘下の指導員に対しても同様の規則を遵守するよう監督を行います。
このプライバシー・ステートメントは適宜改訂されることがあります。当社は、このプライバシー・ステートメントを改訂する場合には、 従前のプライバシー・ステートメントに代えて改訂後のプライバシー・ステートメントをホームページに公表します。
当社は、クライアント各位の個人情報の適切な取扱いを向上させるべく、このプライバシー・ステートメント、 安全管理措置その他当社が関与する個人情報に関する活動を適宜見直すよう努めます。

(“MFA Japan”) is committed to protecting the confidentiality and security of the personal information of clients (“Clients”) who enroll in or attend MEDIC First Aid training programs (‘Courses’) offered by MFA Japan or its instructors (“Instructors”), We believe it is important to continually review and work to protect the confidentiality and security of the personal information of clients (“Clients”) who attend or participate in MEDIC First Aid first aid training programs (“Courses”).
The personal information we obtain from the client is: the client’s name, address, date of attendance, name of the program attended, name of the instructor, and in the case of continuing education courses, we may need to obtain the name and date of any previous courses (including courses offered by other companies) attended. This personal information will be collected by the instructor in a lawful and fair manner prior to the start of the course or before the end of the course and will be submitted to us at a later date.
In order to protect personal information as defined in the “Act on the Protection of Personal Information”, we have established this Privacy Statement as our basic policy and pledge to protect our clients’ privacy as follows

We will handle clients’ personal information only to the extent necessary to achieve the following purposes of use.
- For record keeping: to record who took which course, when, where, and with which instructor, for use in after-sales services to clients and for internal review.
- To send out information postcards to inform clients of the approaching expiration date of their own completion cards.
- For random surveys conducted as part of our Quality Assurance program to ensure and maintain the quality of the courses offered. In this case, the objective is consumer protection.
- For market research and data analysis conducted by the Company to develop educational materials and improve services.
- To communicate with clients themselves who have applied for the Good Neighbor Award. When we disclose your experience as a Good Neighbor on our website, we will do so with the consent of the client himself/herself.
- To post the name of the client’s company, organization, group, or facility on our website for the purpose of promoting first aid with the ultimate goal of a safe community. In such cases, we will only do so with the client’s own approval.
- To respond to various requests by the client himself/herself, including inquiries and requests for card reissuance.
- To conduct sales or marketing activities for the client himself/herself, including the introduction of continuing education courses, new programs, or products that may be of benefit to the rescue at the citizen level.
- All other purposes for the appropriate and smooth performance of transactions and after-sales services with the client by the Company and the client’s own instructor.
Personal information of a client may be used by the instructor in charge of the client as an instructor within the scope necessary to achieve the aforementioned purposes of use. In such cases, we shall exercise necessary and appropriate supervision over such instructor in accordance with relevant laws, regulations, guidelines, etc., in order to ensure the safe management of the client’s personal information.
We will not disclose or provide clients’ personal information to third parties without the client’s own consent, except when required to do so by judicial or administrative authorities. We also supervise our affiliated instructors to ensure that they comply with the same rules.
This Privacy Statement may be revised from time to time. Whenever we revise this Privacy Statement, we will publish the revised Privacy Statement on our website in place of the previous one.
We will endeavor to review this Privacy Statement, security measures, and other personal information-related activities in which we are involved from time to time in order to improve the appropriate handling of personal information by our clients.
